Adult Roller Hockey Forfeit Submission form

By completing the form below, you are confirming that your team will be forfeiting the scheduled game that you are submitting. Once this form is submitted that forfeit cannot be overturned. You and your team are also required to pay the $100 forfeit before your team's next scheduled game. If the fee goes unpaid past your next scheduled game the team captain will have their credit card on file charged for the $100 fee.
